The Post School Outcomes initiative focuses on preparing students with disabilities for successful transition from school to post-secondary education, employment, and independent living.
Transition Planning
Effective transition planning begins early and involves collaborative efforts between students, families, schools, and community partners to ensure students are prepared for life after high school.
Key Components
- Career Exploration: Helping students identify interests, strengths, and career goals
- Work-Based Learning: Internships, job shadowing, and community-based instruction
- Self-Determination: Teaching students to advocate for themselves and make informed decisions
- Independent Living Skills: Daily living skills necessary for adult independence
- Post-Secondary Education: Preparation for college, vocational training, and continuing education
- Employment Preparation: Job skills, workplace behaviors, and career readiness
Graduation and Success
Our goal is to increase graduation rates and improve post-school outcomes for all students with disabilities through evidence-based transition practices and comprehensive planning.
